4. Priority data (if priority is claimed)

6. Priority documents (if priority is claimed), to be filed within two months from the filing date

4. Power of Attorney, no notarization

1. Powers of Attorney in the name of the Assignor and the Assignee, no legalization

2. Original Assignment (or its Certified copy) or a signed and sealed Assignment form, notarization is required
